Forms and Waivers

Please acknowledge your understanding of each of the 9 Code of Conduct items listed below.
This Code of Conduct is in effect from the time of departure on Friday, September 6, 2024 until
the buses return on Sunday, September 8, 2024.

1. This trip is hosted and produced by Campus Changer Network and is centered around building authentic relationships through adventure and team competition. Healthy competition is encouraged. That promotes tribe unity and respect for both your teammates and the other tribes competing. Any disrespectful behavior within or between tribes will not be tolerated including foul language, degrading gestures or imagery, vandalism or property theft, and slanderous or gossiping speech towards any other participants, trip leaders, or volunteers.

2. Alcohol, tobacco, weapons, vapes and illegal drug use are prohibited during the course of the Survivor trip.

3. Curfew: Everyone must be inside their tents by 11:00pm, unless otherwise directed by trip leaders.

4. Everyone must be in bed with lights out by 12:00am, unless otherwise directed by trip leaders.

5. For the safety of everyone involved, men are not allowed in women's tents and women are not allowed in men's tents. If you prefer other accommodations, please notify us.

6. Ensure someone else on your team knows where you are at all times. Do not go anywhere alone (the bathroom is an exception).

7. Directions from Survivor staff, survivalists, mentors, and volunteers must be followed.

8. Don't do anything to jeopardize your personal health or safety.

9. Failure to follow this Code of Conduct will result in participant(s) being sent home on the participant's expense.

Code of Conduct Agreement - read statement below:
I understand that if I violate any of these rules I could be sent home immediately according to the Campus Changer Network discretion. If I witness any violations of the above code I will report it to my room leader or tribe leader immediately. I also understand that if I am sent home
for violating this code, I will be responsible for paying for my own transportation back home.

Please acknowledge your understanding of and agreement to each of the 3 statements below.

1. Scope of Event - read statement below:
Survivor is an event hosted and produced by Campus Changer Network. The phrase "Survivor" (hereafter "this event") encapsulates all activities and transportation planned
by Campus Changer Network from time of departure on September 6, 2024 through September 8, 2024.

2. Bodily Harm Waiver - read statement below:
I am aware that the activities of this event increase the chance of bodily harm, including serious injury, sickness and death. I also am aware there will be flying objects (such as drones) used to capture media at any given time that also increase the chances of bodily injury. I do not hold Campus Changer Network or the volunteers of this event responsible for any bodily harm that occurs at this event, including serious injury, sickness and death. Therefore I agree without reservation that neither I, nor any party on my behalf, will pursue legal action against Campus Changer Network or this event's volunteers for any bodily harm, including serious injury, sickness and death, which could occur at Survivor.

3. Personal Possessions Waiver - read statement below:
The group setting of this event provides many opportunities for personal possessions to be lost, broken, or stolen. I do not hold Campus Changer Network or this event's volunteers responsible for any personal possessions that are lost, stolen, or broken at this event. Therefore I agree without reservation that neither I, nor any party on my behalf, will pursue legal action against Campus Changer Network or this event's
volunteers regarding any personal possessions that are lost, stolen, or broken at this event.

By signing below, I agree to the statements above.

I hereby grant permission to the rights of my image, likeness and sound of my voice as recorded on audio or video without payment or any other consideration. I understand that my image may be edited, copied, exhibited, published or distributed and waive the right to inspect or approve the finished product wherein my likeness appears. By signing this release I understand this permission signifies that photographic or video recordings of me may be electronically displayed via the Internet or print media. This release applies to photographic, audio or video recordings collected during Survivor, from September 6, 2024 to September 8, 2024. By signing this form I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release any and all claims against any person or organization utilizing this material.
